Bij deze stoornis zijn hele categorieën mentale representaties afwezig uit het psychisch apparaat. Aangenomen wordt dat het kind zich door deze “mental block” beschermt tegen specifieke, al te pijnlijke representaties, die het gevolg zouden zijn van mentale processen. Mentale processen worden dan zelf afgeremd. Het verschil tussen mentaal proces en mentale representatie wordt wel als volgt uitgelegd: als een mentale representatie een melodie is, dan is het mentale proces de viool waar de klank uit voortkomt (Fonagy e.a., 1993).
